ATTENTION PARENTS,
We will have two (2) options for signing day. You have a choice between June 15th or June 16th at either Chestnut Log or E. Carrollton Park at 6:30 pm.
Please complete and bring your payment plan form . We will be taking cash, check or credit card. Bring a copy of your player's birth certificate if you do not have one on file with us already.
Once we have your payment plan form, your player will receive his/her access code to register online in the affinity system.
